The VI1 Worksheet provides a form for describing the identity of the wines, producer and the exporting agent. The completed worksheet must be sent to ETS for the proper completion of VI1 documents.

This form establishes the authenticity of the information in the VI1 Export Worksheet. The form needs to be signed by a representative of the winery and attached to the information submitted by ETS Laboratories to the regulatory authorities.
